- Beneficiary Form
- This form allows you to change your designated beneficiary in the event of your death.
- Member I.D. Form
- This form is only for new employees. Most employees should get this form from their employers. In the event that you do not receive this form from your employer, you can download it here.
- Refund Form
- This form is for SPTRFA members who are no longer contributing to SPTRFA. These members have the option of taking a refund of their contributions. For other options, please see the Separating from Service brochure, available in our Brochures area.
