Beneficiary Form

This form allows you to change your designated beneficiary in the event of your death. According to law, you cannot change your designated survivor for a survivor benefit.

Federal Income Tax Withholding Form

This form allows you to change the amount withheld from your monthly benefit for federal income taxes.

Minnesota State Income Tax Withholding Form

This form allows you to change the amount withheld from your monthly benefit for Minnesota state income taxes.

Direct Deposit Form

This forms allows you to start monthly electronic deposits of your benefit amount. The form can also be used for changes to the account your benefit is deposited to.
